The process to change NPI information is very easy. Go to NPPES Website.

Sign In or Create an Account

You can either create a login ID or if you already have an NPI login account then simply click the “Sign In” link.

To create an NPI login you simply have to click on the “Create or Manage Account” link. If you want to create an NPI login for an individual health care provider then you must enter the reverse NPI number, first and last name, social security number, and date of birth. If you want to create an account for an organization then you have to enter the NPI number, organization name, and Employer Identification Number (EIN).

Click “View/Modify NPI Data”

After login inside you simply have to click “View/Modify NPI Data.” Now you will get an option to enter your new address and then click “Submit.”

Change NPI Address Information by Mail

The other option to change NPI address information is by Email. By calling the NPI Enumerator at 800-465-3203 you can request them for a form by mail. You can also download the NPI application/update form from the CMS forms page.

Write the NPI Number

On the NPI update form, you should check box 2and write the NPI number in the space provided. Now check the “Change of Information” box.

Enter the New Address

Now go to section 3 of the NPI application and then you should enter your new address details. Give a new residential address in part A only if it is also your mailing address for the company. Give a new residential address in part B only if it is also your place for business practice.

Enter Contact Information

In section 5 you must enter your contact details.

Send the form

When you fill in all your new information details on the Application form, it’s time to recheck all the information and then send it to NPI Enumerator P.O. Box 6059 Fargo, ND 58108-6059.
